Introduction

To strengthen community support on end of life care, home-based care support and series of life and death public education was launched by TWGHs Enhance Home Care and Community Care Services (Shatin). Project provides on-site support to service users and carers with end of life care needs, and deliver latest information to public.

Service Scope

• Conduct family meeting to conflicts regarding End of life topic
• Provide Psychological and emotional support for both service user and carer
• Symptom management on Terminal illness
• Home care support services to relieve carer stress
• Nursing care
• Consultation on Advance Care Directives (ACP) and Advance Directives(AD)
• Organize therapeutic group to promote self reflection and relieve carer stress
• Deliver community talks to raise public awareness on End of Life topics
