社區支援服務
「健康頤年」及「原居安老」是本院社區支援服務的重點。「健康頤年」是指健康生活、有效控制疾病及保持身心健康，包括抱有積極態度參與社會和面對人生最後的階段。而「原居安老」則為長者原地提供適切的支援服務，讓他們即使在健康狀況有變的情況下，仍能繼續在他們熟悉的社區內居住，頤養天年。故此，本院積極發展社區支援服務，透過以下共45個不同類型的服務單位提供多元化及優質的服務。

Introduction
“Farewell Home Care 3.0” aims to provide home-based support services for elderly persons and their families who require end-of-life care and palliative services. Using a Co-Care and cross-professional case management model, the program offers comprehensive support for elders who wish to “dying-at-place,” and assists them and their families in formulating an Advance Care Plan. Through our home care services team, the program helps elderly persons in the community with end-of-life needs and financial difficulties to plan ahead, and provides high-quality end-of-life care according to their wish to “dying-at-place,” thereby granting greater autonomy in the final stage of life.
Service Scope
- Promote Advance Care Planning to elders and their families
- Provide Bereavement support and after-death arrangements
- Conduct family meeting to conflicts regarding End-of-life topic
- Provide Psychological and emotional support for both service user and carer
- Symptom management on Terminal illness
- Home care support services to relieve carer stress
- Nursing care
- Consultation on Advance Care Directives (ACP) and Advance Medical Directives (AMD)
- Organize therapeutic group to promote self-reflection and relieve carer stress
- Deliver community talks to raise public awareness on End-of-Life topics
