Jockey Club End-of-life Community Care Project: Unison (TWGHs)


With funding from the Hong Kong Jockey Club Charities Trust, the Tung Wah Group of Hospitals Rehabilitation Services Section launched a five-year program titled “The Jockey Club End-of-Life Community Care Project: Unison” (JCECC: Unison), starting in August 2024. This initiative, led by the Jockey Club, integrates research and training from the University of Hong Kong and the Chinese University of Hong Kong aiming to establish care service standards for individuals with disabilities in the terminal phases of illness and their families. 

In its first year, the project supported 160 persons with disabilities (PWDs) and their families who had End-of-Life care needs, offering medical care, psychosocial and spiritual support, and practical assistance. In addition, it trained 580 professional and frontline caregivers in palliative care skills and recruited 47 volunteers. Through a variety of life and death education activities and promotion materials, we reached over 3,900 individuals. Meanwhile, comfort care rooms established in residential homes for PWDs, allowing residents who wish to pass away in the homes to maintain their dignity and quality of life until the end of their life journey.
