Introduction

“Farewell Home Care 3.0” aims to provide home-based support services for elderly persons and their families who require end-of-life care and palliative services. Using a Co-Care and cross-professional case management model, the program offers comprehensive support for elders who wish to “dying-at-place,” and assists them and their families in formulating an Advance Care Plan. Through our home care services team, the program helps elderly persons in the community with end-of-life needs and financial difficulties to plan ahead, and provides high-quality end-of-life care according to their wish to “dying-at-place,” thereby granting greater autonomy in the final stage of life.

Service Scope

• Promote Advance Care Planning to elders and their families
• Provide Bereavement support and after-death arrangements
• Conduct family meeting to conflicts regarding End-of-life topic
• Provide Psychological and emotional support for both service user and carer 
• Symptom management on Terminal illness
• Home care support services to relieve carer stress
• Nursing care
• Consultation on Advance Care Directives (ACP) and Advance Medical Directives (AMD)
• Organize therapeutic group to promote self-reflection and relieve carer stress 
• Deliver community talks to raise public awareness on End-of-Life topics
