History

Established in March 2001, Wong Cho Tong Social Service Building is named after benevolent Mr. Wong Cho-tong in appreciation of his generous donation of $16.5 million towards the Building's capital cost. Since its inception, the Building has been championing the notion of service integration and providing diversified and value-added social services to the community. With the seamless collaboration of the service units housed in Building, Wong Cho Tong Integrated Home Care Service offers the public the convenience of community services and actualize the concepts of “ageing in place” and “continuum of care”.

Objective

  • Home Support Services (HSS) and Integrated Home Care Services (Frail Cases) aim to provide holistic services to the target service users living in the community to facilitate their continuous living at home for as long as possible and achieve or maintain their optimal level of functioning. This service also provides support services to their carers to alleviate their care stress. Tailoring the services in accordance with individual elderly user’s need, the service teams would actualise the concepts of “ageing in place” and “continuum of care”.

Service Recipients

  • Home Support Service :
    • Elderly persons : People aged 60 or above who are assessed to be at the state of mild impairment or requiring higher level of care under InterRAITM Check up (CU) assessment tool or assessed and recommended for Home Support Services under the Standardised Care Need Assessment Mechanism for Elderly Services.
    • People with disabilities or individuals and families with social needs: application for HOME SUPPORT SERVICE under these categories have to be referred by registered social workers from social welfare service units.
  • Frail Cases:
    • Aged 60 or above living in the community and not receiving institutional service; and assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services.
    • Carers of the service users of IHCS (Frail Cases)
