Brief History
Lok Kwan Home Care Service is run by Tung Wah Group of Hospitals, aims at providing home care services for Persons with Severe Disabilities living in Hong Kong Island and Islands. The service commences on March 2014.
Service Objective
- To provide a package of home-based support services to meet the needs of persons with physical / intellectual / disabilities to enable their continuous stay in the community and enhance their quality of life.
- To provide carer training and support to enhance their caring capability and relieve the stress of family members/carers of service users
Targets
1. For persons with severe disabilities as below living in Hong Kong Island and Islands.
- Persons with severe physical / intellectual disabilities who are on the waiting list for subvented residential care service of Hostel for Severely Physically Handicapped, Hostel for Severely Mentally Handicapped and Care and Attention Home for Severely Disabled Persons; or
- Persons with severe physical / intellectual disabilities who are assessed to be eligible for the service in accordance with the Assessment Tool for Home Care Service for Persons with Severe Disabilities; or
- Students attending special schools for children with severe intellectual / physical disabilities;
2. Family members / carers of the above-mentioned service users. |
