History

Tung Wah Group of Hospitals Enhanced Home and Community Care Services (Eastern District) received subsidy from Social Welfare Department in March 2001 and commenced operation in April 2001. It provides alternatives apart from placement at residential homes for frail elders to continue living in the community with comfort and dignity while receiving a wide range of home care services.

The Social Welfare Department has regularised the Enhanced Home and Community Services from September 2020.

Objective

Enhanced Home and Community Care Services adopts an integrated mode of services to meet the nursing and personal care needs of frail elders with the aims of enabling them to age at home, maintaining their maximum level of functioning and providing support to carers in order to strengthen family cohesion.

Service Recipients

  • Living in Eastern District
  • Aged 65 or above. Those aged between 60-64 with proven needs can also apply.
  • Elders assessed to be of moderate or severe level of impairment through the application of Standardized Care Need Assessment Mechanism for Elderly Services, and have need for a comprehensive package of home care and community support services
