History
According to the 2023 Policy Address, the government has established a Designated Team for Special School Leavers since April 2024 at 21 District Support Centres for people with disabilities across Hong Kong. This team is a community support service under the Tung Wah Group of Hospitals Lok Yung District Support Centre (Islands).
Objective
Designated Team for Special School Leavers attached to the centre provides early intervention and extra care and support services to the special school leavers and their caregivers with a view to smooth transition from school to community living for special school leavers and to alleviate the stress of their caregivers.
Service Recipients
The Special Schools paired with our unit are TWGHs CHI-LI PAO School and Hong Chi Shiu Pong Morninghope School.
- Students who will be leaving school within the next 6 months
- Boarding students who will be leaving school within the next 12 months
- Graduates who have left school for no more than 18 months
- Caregivers of the above school leavers
