History
According to the 2023 Policy Address, the government will establish a Designated Team for Special School Leavers in April 2024 at 21 District Support Centres for people with disabilities across Hong Kong. This team is a community support service under the Tung Wah Group of Hospitals Lok Kwan District Support Centre.
Objectives
Designated Team for Special School Leavers attached to the centre provides early intervention and extra care and support services to the special school leavers and their caregivers with a view to smooth transition from school to community living for special school leavers and to alleviate the stress of their caregivers.
Target
The Special Schools paired with our unit are TWGHs Tsui Tsin Tong School and TWGHs Kwan Fong Kai Chi School.
- Students who will be leaving school within the next 6 months
- Boarding students who will be leaving school within the next 12 months
- Graduates who have left school for no more than 18 months
- Caregivers of the above school leavers
