
Lok Kwan District Support Centre – Designated Team for Special School Leaver
Designated Team service includes social workers, therapists, nurses, and rehabilitation worker who provide the following services:
1. Lectures and Activities
- Organizing lectures, programs, events, or workshops to provide information on community support services and resources.

2. Groups and Training
- Offering mutual support groups and training courses for caregivers to enhance their caregiving and interpersonal skills.
- Providing personal development, social, and recreational groups or therapeutic training for school leavers.

3. Case Management
- Delivering case management services, including case counseling, emotional support, setting short-term and long-term training plans, matching suitable community support services, and referrals.
- Regularly contacting cases through home visits, school visits, outreach, and case discussions to assist and follow up on school leavers' adaptation to community life.
