History
CareWise is the first carers support center of TWGHs, and it began providing services in October 2023. The center aims to become a reliable partner for carers, offering various forms of support including carer’s case management services, counselling services, respite services, skill training sessions to carers, and rehabilitation equipment rental services, etc. CareWise also connects with other community care service units to construct an all-encompassing support network for carers, committing to support, learn, and grow together with them.
Objectives
- To provide carers with a range of support services to meet their needs at different stages of caring for the elderly.
- To offer carers’ case management services, assess the needs of carers, and develop personalized plans to support their arrangements for elder care and personal life planning.
- To provide respite services and various types of stress-relief activities, allowing carers to have me-time and platforms to take a break and also to deal with personal matters.
- To establish a community support network through different community activities for carers
