History
Tung Wah Group of Hospitals was offered in the bidding for the participation in the Integrated Care and Discharge Support for Elderly Patients (ICDS) of Alice Ho Miu Ling Nethersole Hospital by the Hospital Authority. The program commenced service from 1 January 2012. Through the services provided by the multi-disciplinary professionals, the home support team is committed to provide nursing, rehabilitation, social, personal care and meal service to elderly patients for an average period of 8 weeks, as well as providing training and support services to caregivers. The objectives of the program is not only to enhance early hospital discharge and community-based rehabilitation and support services to those elderly patients discharged from hospital but to provide support and training to caregivers to relieve their stress from post-discharge care of the elderly.
Objective
The service aims at enhancing support to elders discharged from hospitals having difficulties in taking care of themselves and support to their carers at the same time with a view to enhancing the quality of life of the elders, facilitating their stay in the community and reducing the rate of unplanned re-admission to hospital.
Service Recipients
Discharged elderly patients aged 60 or above residing in Tai Po and being referred by Discharge Planning Team. |
